Autoimmunity vs autoinflammation in Behcet's disease: do we oversimplify a complex disorder?

چکیده

Behcet’s disease (BD) is a systemic inflammatory disorder with a diverse spectrum of clinical manifestations including mucocutaneous, ocular, vascular, gastrointestinal, musculoskeletal and central nervous system involvement [1]. A complex genetic background leading to a pro-inflammatory, innateimmune-system-derived activation perpetuated by adaptive immune responses against enviromental and auto-antigens is accepted to be the hallmark of BD [2]. This review aims to make an in-depth critical analysis of current data for recent controversies on the role of innate immune system vs autoimmunity in BD [3–5].

The term autoinflammation was initially introduced to denote a group of diseases that lacked the usual features of autoimmunity (high-titer autoantibodies and antigenspecific T cells), and were subsequently recognized as disorders of the innate immune system. Patients with autoinflammatory disorders present with chronic and recurrent bouts of systemic inflammation that are mediated by the cells...

CTLA4 Gene Variants in Autoimmunity and Cancer: a Comparative Review

Gene association studies are less appealing in cancer compared to autoimmune diseases. Complexity, heterogeneity, variation in histological types, age at onset, short survival, and acute versus chronic conditions are cancer related factors which are different from an organ specific autoimmune disease, such as Grave’s disease, on which a large body of multicentre data is accumulated. For years t...
